To Register - You MUST have an email on file with our office and your contact information must be correct and the same in each of your children's records

 

TO REGISTER:

  • Pick a user name (If reregistering chose one that you have not used before)
  • Enter the email we have on file
  • Enter YOUR first and last name -exactly as we have on file.
  • Enter your birthday (mm/dd/yyyy).
  • This must match exactly what we have on file in the contacts for each of your children. (your name spelled the same way and the same email for all children)

 

A link for setting up a password will be sent to the email we have on file.

 

If you have multiple children you will see all of your children with one user name and password. To change between the children's accounts click on the name in the upper right, then click "select another patient". If you don't see all of your children, call us so that we can update the missing child's contact section.

 

If you get an error message CALL US BETWEEN 8:30 and 5:00 M-F and we will update your information so you can register. If you are a new patient you must call us before registering.
