Register for the portal by using the PARENT first and last name that matches exactly what we have in the contact information for your child's (children's) account(s). You will see each of your children listed with one log on. You can then log into each of their individual accounts with the same user name and password. 

NEW PATIENTS can register once you have an email and contact information on file with our office.

 If you get an error message CALL US BETWEEN 8:30 and 5:00 M-F and we will update your information so you can register. If you are a new patient you must call us before registering.
